New Melanoma Treatment Might Delay Cancer Progression - Drugs ...

SATURDAY Sept. 29, 2012 -- Researchers say they've discovered a two-drug combination that delays treatment resistance in patients with advanced melanoma.

By targeting different points in the same growth-factor pathway, the kinase inhibitor drugs dabrafenib and trametinib postponed the development of drug resistance in patients with BRAF-positive metastatic melanoma, the study authors said.

Melanoma is the most serious, and often deadly, form of skin cancer. In about half of patients with melanoma that has spread, tumor growth is caused by genetic mutations that keep the BRAF protein -- part of the MAPK cell growth pathway -- constantly activated. Drugs that inhibit BRAF activity can rapidly stop and reverse tumor growth in about 90 percent of patients. But the response is temporary in most cases, and tumor growth resumes in six or seven months, the researchers explained.

Previous research suggested that this drug resistance develops because the MAPK pathway gets turned back on through activation of MEK, another protein that is part off the MAPK pathway.

"We investigated this (drug) combination because of research we and others have conducted into the molecular underpinnings of resistance to BRAF inhibitor therapy," study lead author Dr. Keith Flaherty, of the Massachusetts General Hospital Cancer Center, said in a hospital news release.

The phase 1 and 2 study was sponsored by GlaxoSmithKline, which developed both drugs.

"We found that adding the MEK inhibitor trametinib to BRAF inhibitor dabrafenib clearly delays the emergence of resistance. In fact, the combination was at least twice as effective as BRAF inhibition alone," he said.

One expert agreed that the results were encouraging.

"This study addresses the problem of resistance to the mitogen-activated protein kinase [MAPK] pathway. This study combined dabrafenib [a selective BRAF inhibitor] and trametinib, [a selective MAPK kinase (MEK) inhibitor]. These drugs can be safely combined, and the survival was significantly improved without untoward side effects," said Dr. Michele Green, a dermatologist at Lenox Hill Hospital in New York City.

The study was conducted at 14 sites in the United States and Australia and included 162 patients who received different dose combinations of the drugs: two daily 150 milligram (mg) doses of dabrafenib plus one 2 mg dose of trametinib; the same dabrafenib dose with a 1 mg dose of trametinib; or treatment with dabrafenib alone.

Patients who received dabrafenib alone were able to receive the full-dose combination treatment if their cancer resumed progression.

Treatment with both combinations of dabrafenib and trametinib led to a four-month longer delay in drug resistance than treatment with dabrafenib alone. After one year of treatment, 41 percent of patients receiving the full-dose combination treatment had no progression of their melanoma, compared with 9 percent of those receiving dabrafenib alone.

Among patients receiving the combination treatment, the development of side effects such as skin rash and a less dangerous type of skin cancer called squamous cell carcinoma was similar to that typically seen in patients taking only one of the drugs.

The study was scheduled for presentation Saturday at the European Society for Medical Oncology meeting in Vienna and simultaneous publication in the New England Journal of Medicine.

The drug combination is now being tested in a larger phase 3 study, which is required for U.S. Food and Drug Administration approval.

This paper describes how the performance of AI machines tends to improve at the same pace that AI researchers get access to faster hardware. The processing power and memory capacity necessary to match general intellectual performance of the human brain are estimated. Based on extrapolation of past trends and on examination of technologies under development, it is predicted that the required hardware will be available in cheap machines in the 2020s.

Computers have far to go to match human strengths, and our estimates will depend on analogy and extrapolation. Fortunately, these are grounded in the first bit of the journey, now behind us. Thirty years of computer vision reveals that 1 MIPS can extract simple features from real-time imagery--tracking a white line or a white spot on a mottled background. 10 MIPS can follow complex gray-scale patches--as smart bombs, cruise missiles and early self-driving vans attest. 100 MIPS can follow moderately unpredictable features like roads--as recent long NAVLAB trips demonstrate. 1,000 MIPS will be adequate for coarse-grained three-dimensional spatial awareness--illustrated by several mid-resolution stereoscopic vision programs, including my own. 10,000 MIPS can find three-dimensional objects in clutter--suggested by several "bin-picking" and high-resolution stereo-vision demonstrations, which accomplish the task in an hour or so at 10 MIPS. The data fades there--research careers are too short, and computer memories too small, for significantly more elaborate experiments.

There are considerations other than sheer scale. At 1 MIPS the best results come from finely hand-crafted programs that distill sensor data with utmost efficiency. 100-MIPS processes weigh their inputs against a wide range of hypotheses, with many parameters, that learning programs adjust better than the overburdened programmers. Learning of all sorts will be increasingly important as computer power and robot programs grow. This effect is evident in related areas. At the close of the 1980s, as widely available computers reached 10 MIPS, good optical character reading (OCR) programs, able to read most printed and typewritten text, began to appear. They used hand-constructed "feature detectors" for parts of letter shapes, with very little learning. As computer power passed 100 MIPS, trainable OCR programs appeared that could learn unusual typestyles from examples, and the latest and best programs learn their entire data sets. Handwriting recognizers, used by the Post Office to sort mail, and in computers, notably Apple's Newton, have followed a similar path. Speech recognition also fits the model. Under the direction of Raj Reddy, who began his research at Stanford in the 1960s, Carnegie Mellon has led in computer transcription of continuous spoken speech. In 1992 Reddy's group demonstrated a program called Sphinx II on a 15-MIPS workstation with 100 MIPS of specialized signal-processing circuitry. Sphinx II was able to deal with arbitrary English speakers using a several-thousand-word vocabulary. The system's word detectors, encoded in statistical structures known as Markov tables, were shaped by an automatic learning process that digested hundreds of hours of spoken examples from thousands of Carnegie Mellon volunteers enticed by rewards of pizza and ice cream. Several practical voice-control and dictation systems are sold for personal computers today, and some heavy users are substituting larynx for wrist damage.

The Human Spam Filter Effect ? Posting Too Frequently

If you?ve ever used an email client before you know about spam filtering ? it?s an automated mechanism that identifies spam-like content and throws it straight into the trash bin. Many Facebook users are so in-tuned with internet browsing that they?ve developed their own built in spam filter and are quick to discard overbearing, unwelcomed, and useless status updates. In fact, people get so aggravated by a high frequency of posts that they don?t even want you having access to their wall anymore, and subsequently they unlike your page.

So how often should you post?

This is no rocket science and there is no magic formula. Each business is different so it's all about A/B testing. Besides, your emphasis should be on quality not quantity. The general rule of thumb is that if you don?t have something great to post, don?t post at all. With that said, if you?re taking the time to research your market and pay attention to the requests, opinions, and interests of fans, you should be able to produce at least 3-5 good status updates per week to start with. Increase the frequency if you see a positive response and increased engagement from your fanbase. Again, test different scenarios and don't be afraid to fail at times...

Maybe it was a discount, some exclusive content or app, some genuinely helpful information, or some wholesome entertainment that made them come back for more. Whatever it was, you?re obviously not doing it anymore. Keeping fans on Facebook is a lot like keeping fans with any other type of media. If a television show is no longer entertaining, viewers stop watching, ratings decrease, and eventually it gets taken off the air. If you?re noticing a steady decline in fan numbers, you need to take action immediately and reintroduce the incentive-based status update strategy. A good way to let your fans know that they?re appreciated is to use a reveal tab/fan gateway to give them access to exclusive content that only fans can see. This will make your fans feel valued and motivate them to keep following your statuses to avoid missing any upcoming contest or giveaway. It will also encourage non-fans to join your page out of curiosity.

Failed Initial Targeting

Maybe the reason why they?re unliking you is because they were never really your fans to begin with? It could be that in your rush to get as many fans as possible in the beginning, you may have neglected the process of carefully targeting your audience. There are lots of ways to get fans on Facebook, including buying them from marketing gurus that are equipped with all the latest tools and tricks. However, if you don?t make an effort to ensure that your fans are properly targeted you could find a large percentage of them losing interest down the road.

So how do you do a better job of targeting your true fan base?

One of the best "organic" ways to attract fans that are likely to stick with you for the long haul is to advertise your fan page EVERYWHERE you can. Think beyond your website, e.g. email signatures, business cards, flyers, invoices, brochures etc... Another great way to attract targeted fans is do joint ventures with other fan pages in your niche. The concept is simple ? you find already successful fan pages that have a lot fans who would probably be interested in your page, then you contact the page administrators and ask them what it would take for them to post a link to one of your statuses in their next update. However, the most efficient ways to attract laser targeted Facebook fans will require you to put your hand in your pocket: Facebook Ads! Using Facebook Ads you can target people by location, age, sex, marital status and most importantly INTERESTS. This gives you the ability to display your ad only on profiles that are likely to become fans and/or customers. Coupled with a fan-gated contest or giveaway, Facebook Ads is ridiculously efficient! Try different ad rotations and targeting options to ensure you get the best bang for your buck.

Your Posts Just Suck

There really is no better way to say it. Your fans are unliking your page for the simple reason that they are tired of reading substandard posts that lack useful information or entertainment value.

How do you fix this?

In essence, you need to learn how to post status updates that are captivating and share-worthy. Adding a dislike button to each one of your statuses using Status Magic (as explained in the previous link) might be a good way to find out what your fans do not like before they get so sick of your antics that they completely unlike your entire page.

Use media to your advantage.

If you don?t have the resources, time, or skill to create your own quality media content, find it elsewhere on the web and introduce it to your fans. Sometimes being a known resource and sharing content that has already been created is all it takes. You?d be surprised how many people will stick around just because they know you always bring the best of the web to their fingertips. Think of websites like Digg or Reddit to find potential "viral content" that is relevant to your niche.

Use news and trends to your advantage.

Again, you don?t have to be a world-class journalist and conduct interviews or author breaking news. You just need to point out the latest trends and occurrences that your fans are likely to be interested in, and maybe add your own spin or opinion on the matter. Remember that your fans are basically subscribers; keep them up-to-date and involved.

Be interactive and engaging.

When you?re posting a status about an upcoming promotion, recent news, or a popular trend, make sure you pursue the input of your fans. Hold a poll, ask them to leave a comment, and give them the opportunity to engage in open dialect. Getting your fans involved is a good way to keep their mouse cursor away from the unlike button, and it also creates a buzz. If you're running out of ideas on what to post, check out this article.

And finally...Have a personality and be personable!

Don?t be afraid to be funny, witty, and unique. Stand out from the crowd by acting like a human being, and not a status updating advertising robot. There is no preset formula that will make people like you. Facebook is a digital platform but it still operates in the real world; people like other people simply because... they?re likable. What?s the point of starting a page on a social network if you?re just going to be antisocial?

?British designer Katherine Hooker will debut her latest collection of coats and jackets this October with the opening of a U.S. pop-up shop in New York on Oct. 5. The pop-up store, which will be located in New York's SoHo neighborhood at 436 W. Broadway, will be open through Oct. 14. The shop will be the designer's only bricks-and-mortar retail presence outside of her flagship in London's Chelsea neighborhood. The shop will stock Hooker's jackets, coats, vests, and ready-to-wear.

Top 8 Debate Zingers

Almost all presidential and vice presidential debates include good lines. But not every debate includes a real zinger: a rejoinder that nails the opponent and sticks in viewers? minds for weeks to come. Here are eight of National Journal?s favorites.

1. ?You are no Jack Kennedy.? Sen. Lloyd Bentsen, D-Texas, cut a dour figure onstage at the 1988 vice presidential debate in Omaha. But that evening he also delivered one of the best put-downs in political debate history, in response to a claim by Sen. Dan Quayle of Indiana?that ?I have as much experience in the Congress as Jack Kennedy did when he sought the presidency.? Speaking slowly and deliberately, Bentsen replied, "Senator, I served with Jack Kennedy. I knew Jack Kennedy. Jack Kennedy was a friend of mine. Senator, you are no Jack Kennedy.? The audience burst into applause. ?That was really uncalled for, senator,? Quayle said, with a stony face. ?You?re the one who was making the comparison, senator,? Bentsen replied.

?2. ?There you go again.? Here?s a line so well-received that current GOP nominee Mitt Romney has said he?d like to use it against President Obama. It landed at the only televised debate between Ronald Reagan and President Carter. ?Governor Reagan, as a matter of fact, began his political career campaigning around this nation against Medicare,? Carter said; Reagan laughed and shook his head. Carter wrapped up his pitch for a program of national health insurance, and it was Reagan?s turn to reply. ?Governor. There you go again,? Reagan said, and explained that he?d merely been a proponent of a different piece of legislation. It was such a good line that Reagan reprised it in his debate with former Vice President Walter Mondale in 1984.

3. ?Senator Dole has richly earned his reputation as a hatchet man.? A question over President Nixon?s pardon led to a rather dark exchange between Sens. Bob Dole and Walter Mondale during the first-ever vice presidential debate, held in Houston in 1976. Dole said he wouldn?t be addressing Nixon?s pardon because ?it?s not a very good issue??as done and dusted as the wars in Vietnam, Korea, and the two World Wars, which he characterized as ?all Democrat wars.? Mondale responded, ?Senator Dole has richly earned his reputation as a hatchet man tonight.? Dole?s odd allegation?and Mondale?s pithy response?became the most remembered moment of the debate.

4. ?If Bush keeps it up, he's going to be the Joe Isuzu of American politics.??Both Vice President George H.W. Bush and Massachusetts Gov. Mike Dukakis managed to get in a few zingers during the 1988 presidential debate in Winston-Salem, N.C. Dukakis claimed Bush couldn?t be serious about bringing down the deficit, because the Republican wanted both more defense spending and lower taxes. ?If he keeps this up he's going to be the Joe Isuzu of American politics,? Dukakis said, referring to a '70s car salesman famous for his over-the-top TV ads. ?Is this the time to unleash our one-liner? That answer was about as clear as Boston Harbor,? Bush retorted. ?

5. ?I?m going to try to help you do that, Joe.? Dick Cheney didn?t miss a beat when then-Democratic Sen. Joe Lieberman brought up Cheney?s private-sector career during the 2000 vice presidential debate at Kentucky?s Centre College. ?I'm pleased to see, Dick, from the newspapers that you're better off than you were eight years ago, too,? Lieberman said. ?I can tell you, Joe, the government had absolutely nothing to do with it,? Cheney responded, to laughter. Missing the chance to jab Cheney over his Halliburton connections, Lieberman said, ?I can see my wife and I think she's thinking, ?I wish he would go out into the private sector.' ? Cheney dryly remarked, ?I?m going to try to help you do that, Joe??a zinger that ended the exchange.

6. ?I am not going to exploit for political purposes my opponent?s youth and inexperience.? When President Reagan ran for reelection in 1984, he was already the oldest president in American history?a fact raised during a presidential debate on national-security issues. Reagan told Baltimore Sun reporter Hank Trewhitt, a panelist, that there was no doubt in his mind he had the capacity to be commander in chief. ?Not at all, Mr. Trewhitt, and I want you to know that also I will not make age an issue of this campaign. I am not going to exploit for political purposes my opponent?s youth and inexperience.?

7. ?Who am I? Why am I here?? Ross Perot?s running mate in 1992, retired vice admiral James Stockdale, was a Vietnam war hero who joined Perot on the independent ticket essentially as a personal favor. At the 1992 vice presidential debate, Stockdale?s opening statement was almost painfully honest: ?Who am I? Why am I here?? he asked. ?I?m not a politician, everybody knows that.? The Medal of Honor recipient?s oddball opening line drew laughs, but it also brought front-and-center the question of what he was doing on the vice presidential ticket.?

8. ?Where?s the beef?? Presidential and vice presidential debates aren?t the only places where a candidate can work in a zinger: The 1984 Democratic debate will forever be remembered for one pop-culture reference, landed by former Vice President Walter Mondale. After Sen. Gary Hart wrapped up touting a long list of his new ideas, Mondale responded, ?When I hear your new ideas, I?m reminded of that ad: Where?s the beef?? Fresh in many minds at the time was fast-food chain Wendy?s series of commercials in which an elderly woman poses?that question when she is presented with a burger that?s all bun.

College Degrees No Longer Required For Good Middle Class Jobs ...

It's a scary world out there for young people looking to start their careers, and this has led to plenty of college education panic. Don't have a degree, say a host of hyperventilating pundits and extremely anxious parents, and you'll almost certainly be consigned to low-paid wage slavery and a stunted life. Weep, weep.

And while the data certainly show a large and growing gap between the earnings of those with a four-year degree and those without, and no one would argue with your worthy high school guidance counselor's advice that it's generally a good idea to go to college, all this hand-wringing can have the side effect of suggesting ALL IS LOST if somehow getting a bachelor's degree failed to work out for you.

Not so, says a recent study from the Georgetown University Center on Education and the Workforce and Civic Enterprises. The research found that despite the obvious erosion of many traditional routes to middle-class earnings for non-graduates (nope, I wouldn't bet on that factory re-opening in town), there are still 29 million jobs in America that pay middle-class wages (defined here as between $35,000 and $75,000 annually) that don't require a bachelor's degree. Forty percent of those pay more than $50,000 a year. (For New Yorkers and inveterate high achievers, $50,000 might not sound like a lot, but it beats the U.S. median.)

While in the past, blue-collar gigs were the route to the middle class for non-graduates, this study reveals that most of today's jobs for non-grads are now in service sectors like healthcare or white-collar office jobs.

So how do you find your way to one of these reasonably decent jobs if you missed the boat on college? The research suggests getting one of these gigs still takes some effort, with the center suggesting five paths to this sort of steady employment:

  • Associate's degrees. Half of associate's degrees are related to career-oriented fields, such as nursing, business, and information technology.

  • Postsecondary certificates have eclipsed associate's and master's degrees as the second most common post-secondary award after the bachelor's degree-about 1 million are awarded each year.

  • Registered apprenticeships. Nine out of 10 apprentices are men and more than half of apprenticeships are in construction.

  • Industry-based certifications such as Microsoft, Cisco, and CompTIA certifications are test-based postsecondary credentials awarded by employers.

  • Employer-based training represents the largest pathway at $454 billion of spending.

Of course, while it's great to get some counterweight to the general cultural gloom hanging over those who fail to make it out of a four-year college program, the authors of the study don't think enough is being done to help those who need to find a non-college route to a good job.

"Compared to other advanced economies, the United States under-invests in sub-baccalaureate, career and technical education," said Anthony P. Carnevale, the Center's Director and the report's lead author.
